The Healthy Lifestyles team can offer short opportunistic advice, help you to come up with a plan to change a lifestyle of bad habits, or even undertake a full lifestyle screening – whatever you need.
You often find Healthy Lifestyle Advisors out and about at GP practices and community venues across Telford – so keep a look out for them. You can also contact them to make an appointment:

When you see the Healthy Lifestyle Advisor they will ask you to complete a competition slip which will then go into the draw. Only one entry per person or Twitter account.
The competition will close at 5pm on Thursday 30th November and the winners will be notified via Twitter from Friday 1st December.

Prizes will be offered in the order names are drawn out of the hat. e.g first to be drawn out of the hat will win the Family Panto Tickets.
