Live Well Community Hubs

The Live Well Community Hubs bring together a range of services, organisations and groups, at a local community venue to provide a one stop shop to help YOU to manage your health, wellbeing, social and care needs with support. If you would like to be able to do more, know more, enjoy more and feel better, the Live Well Community Hub is there for you to make small positive changes to a happier and healthier you.   

Each hub is staffed by knowledgeable professionals who can signpost residents to the right services, offer practical advice, and help people navigate the health and social care system.

Citizens Advice

Citizens Advice Telford and the Wrekin are on hand to provide you with information, signposting and to book in appointments for all areas of advice including benefits, debt, energy, housing, employment and more.

Telford and Wrekin CVS

Offering information and support around core CVS services such as the All-Age Carers Centre, the All-Age Autism Hub, SENDIASS and the Sunflower Café. Attending staff will also have knowledge and experience to signpost to other services and organisations.

Support with using a phone, tablet or laptop

Free friendly support to help you get the most from your device. Whether you want to learn how to download and use apps, stay safe online, use email, access your health information and order prescriptions online, the team can guide you step by step. Or pop in for a chat and find out how using technology can help you.

Healthy Lifestyles Service

Healthy Lifestyle Advisors can help you live a healthier life, do more of the things you want to do, and generally improve how you feel. With personalised one to one support from our Healthy Lifestyles Advisors, you can achieve those goals.

Stop Smoking Advisors (from the Healthy Lifestyles Service)

If you’re a smoker, stopping smoking can make a big difference to your health, as well as the health of your family and friends. It can improve your chances of living longer and you’ll soon be feeling the benefits of giving up cigarettes. Even if you’ve been smoking for years, it’s never too late to stop.

The Independent Living Centre

In attendance at the Live Well Hub will be Andy, whose role is to support people to remain safe and independent at home. Andy will be able to offer advice, guidance and information on accessing Adult Social Services, as well as having on display a number of gadgets and equipment for you to try.

Telford & Wrekin Council Armed Forces Outreach (Donnington Hubs only)

Telford & Wrekin Council Armed Forces Outreach aims to provide help and support to all military personnel, veterans and their families. Working with service charities and organisations, the outreach team assists and facilitates to ensure that current and former military personnel have access to the right support. They also have a wide range of recreational activities that take place across the borough including Models for Heroes, art classes, a craft and sewing group, plus regular drop-in sessions.

Social Prescribers are here to empower and connect you to services and resources that can improve your health and wellbeing.

Telford Mind Social Prescribers (Southeast Hubs only)

Social Prescribers are here to empower and connect you to services and resources that can improve your health and wellbeing.  Using a therapeutic approach, they provide practical, helpful non-judgemental support.

The Family Hubs

A member of the Family Hubs Team is joining the Live Well offer in the Southeast of Telford to reach more parents and carers to offer support, signposting and workshop information for local families.

Lingen Davies Cancer Support

Through direct engagement with people of all ages Lingen Davies’ LiveLife cancer Awareness service helps break down the barriers around talking about cancer. The team raises awareness of the importance of early diagnosis, highlights the signs and symptoms to look out for, and provides free resources to support better understanding. Speak to the Lingen Davies team about available screening opportunities and the signs and symptoms to look out for.
